§ 4-49. Application.

(a)

The application for a certificate of qualification shall be in writing and shall contain the following information:

(1)

The full name, sex, date and place of birth, and the marital status of applicant, social security number and driver's license number if any;

(2)

The correct resident address and electronic mail (e-mail) address of the applicant;

(3)

The name of the place of business of present employment, the name of employer and the correct address of the place of employment;

(4)

Name, correct address and the period of employment of the last two (2) places of employment.

(b)

The application shall also include a statement that the applicant possesses the qualifications set forth in sections 4-17 and 4-18.
