§ 35.5-12.1. Franchise applications.

(a)

Application. Applicants for a telecommunications systems franchise shall submit applications in accordance with the terms and conditions herein designated.

(b)

Content of applications. Applicants for a telecommunications systems franchise shall submit to the parish written applications, including a map of the proposed installation of equipment, facilities and plant, services to be provided, a construction schedule and an operations schedule, insurance coverages, the certificate of the applicant's corporate secretary or other appropriate document confirming the authority of person(s) who will sign the franchise and permit related documents to bind the applicant, and other requested information as may be required by the parish.

(c)

Application fee. An applicant for a telecommunications system franchise shall pay an initial non-refundable application fee of two thousand five hundred dollars ($2,500.00). The application fee shall be submitted at the time that all application documents are submitted for the grantor's review.

(d)

Publication costs. The grantee shall pay the costs of publication of the telecommunication system franchise, as such publication may be required by law.
